Health Care for Adults w/ Intellectual & Developmental Disabilities: Toolkit for Primary Care Providers


The toolkit is an adaptation for U.S. use of Tools for the Primary Care of People with DD, which was based on Canadian consensus guidelines and was peer-reviewed and evidence-based. In 2013, with a grant from the Special Hope Foundation, the Toolkit was adapted for U.S. use as a website, also accessible by smartphones and tablets. The Toolkit is aimed at primary care providers but also is useful for other health care professionals, as well as adults with disabilities and their caregivers. http://vkc.mc.vanderbilt.edu/etoolkit/
